Social Worker - Supervised Community Care Liaison (SS01)

JOB SUMMARY:

The incumbent supports psychiatrists and inpatient and outpatient care teams in facilitating the development and implementation of Supervised Community Care (SCC) plans. They provide guidance and support regarding various aspects of creating an SCC plan and work with all SCC stakeholders to ensure the process is followed including completing necessary forms, providing documentation, accessing expert advice, and facilitating the development of a comprehensive community treatment plan that addresses each individual's treatment and support needs. They may refer patients for long-term support services as part of the treatment plan. Related tasks may be assigned to meet operational needs.


REQUIREMENTS:

  • Member in good standing of the New Brunswick Association of Social Workers;
  • Experience in mental health or addiction treatment, an asset;
  • Applied Suicide Intervention Skills Training (ASIST) or or obtain it according to the established schedule;
  • Ability to develop effective partnerships with the community network;
  • Ability to facilitate group sessions for clients;
  • Proficiency with computer tools;
  • Clinical judgment and strong analytical reasoning skills;
  • Leadership and sound decision-making abilities;
  • Proven communication and interpersonal skills;
  • Adaptability and flexibility;
  • Strong organizational skills and ability to set priorities;
  • Valid driver’s licence and availability to use one’s own vehicle for work-related duties;
  • Availability to travel within the designated zone;
  • Ability to work independently and as a team member;
  • Physically able to perform assigned duties;
  • Good previous work history (performance and attendance);
  • Adherence to professional ethics principles, to the Network management philosophy and organizational values;
  • Compliance with the rules of confidentiality established by the Vitalité Health Network.
