Education

• Must have a current practicing registration with their professional association
• Degree in Human Services/Health Sciences or related field
• Recognized degree in nursing , or a social work degree.

Knowledge and Abilities

• Substitute Decision Maker Act, Community Care Branch Ministry of Health
• Knowledge of theories, concepts, intervention strategies for case mgmt practice
• Client confidentiality and the Health Information Protection Act
• Knowledge of the Truth and Reconciliation Commission Calls to Action
• Personal Care Home Act & Regulations, Saskatchewan Health Authority policies. 
• Relevant data, assessment, plan of care/interventions, updates, discharge summ.
• Effective interpersonal relationship skills
• Knowledge of First Nations and Metis history in Saskatchewan
• Cost conscious, quality driven care planning and service delivery
• Highly developed written and oral communication skills
• Openness to teaching and providing educational opportunities.
• Ability to adapt to new techniques and best practices
• Advocacy, negotiation, crisis management and conflict resolution
• Mental Health Act, Child and Family Services Act, Health Care Directive
