Job Description: Under the direction of the Manager (or designate) and as part of the interdisciplinary team, the Assessor Coordinator - Degree is responsible for the full scope of case management services for rehab patients.  This is accomplished through collaboration with the patient, family and other members of the interdisciplinary team.  The Assessor Coordinator - Degree liaises with other services in the SHA where patient repatriation needs to occur, in an endeavor to provide the right service at the right time and in the right place.  This position reports directly to the Manager for daily clinical supervision. This position collaborates with acute care, home care, primary health care, long-term care, mental health and addictions services and an array of government and non-government agencies to assess patients' needs and coordinate services to meet those identified needs. The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation and termination/discharge. case management is patient and family-centered, strength-based, flexible cost-conscious, quality driven and includes advocating for an coordinating resources based on assessed need.

Education:

  • Degree in Human Services or related field (Social Work, Nursing)
  • Must have a current practicing registration with Saskatchewan Association of Social Workers or College of Registered Nurses of Saskatchewan
  • Must have a valid driver's license.  Some assignments may require access to a vehicle for work

Knowledge, Skills & Abilities

  • Knowledge of theories, concepts and intervention strategies key for case management practice.
  • Cost conscious, quality driven care planning and service delivery
  • Knowledge of rehabilitation populations being served
  • Demonstrates an understanding of patient confidentiality and the Health Information Protection Act
  • Knowledge of advocacy, negotiation, crisis management and conflict resolution
  • Knowledge of policies, Procedures, Regulation and Legislation pertaining to area of practice, for example:  Mental Health Act, Child and Family Services Act, Health Care Directive and Substitute Decision Maker Act, Youth Drug and Detoxification Act, Community Care  Branch Ministry of Health, Personal Care Home Act and Regulation, and SHA policies
  • Openness to teaching and providing educational opportunities
  • Effective interpersonal relationship skills and oral and written communication skills
  • Completes standardized documentation which accurately reflects cli8nical practice:
    • Relevant Date
    • Assessment
    • Plan of Care/Interventions
    • Discharge Summary
  • Willingness to adapt to new concepts, techniques and best practices in the academic and clinical fields of social work and health care
  • Knowledge of First Nations and Metis History in Saskatchewan along with an understanding of the Truth and Reconciliation Commission Calls to Action and a commitment to culturally responsive care for all patients and families
