Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

Position Summary: Are you a leader in community-driven integrated care? Do you have a passion for primary health care and system transformation? Please apply for the position of Director of Primary Care Transformation. This role will provide leadership to advance primary care transformation in the North York community. As a member of the Flemingdon Health Centre (FHC) senior team, the new Director will focus on bringing to life integrated primary health care for the communities in North York. The Director will also work closely with the North York Toronto Health Partners (NYTHP) Primary Care Network (PCN) including the Primary Care Co-Leads, Project Coordinator, Digital Support Manager and the Primary Care Advisory Committee. Through a matrix reporting structure, the new Director will work with both FHC and North York Toronto Health Partners to drive integrated primary health care. The Director will lead the continued development of our regional primary care network and implement initiatives to advance team-based primary care services in the North York region. The role will support the region in the operational delivery of our primary care access and attachment strategy. The Director will be involved in implementing the vision of the Primary Care Action Team (PCAT) so that every resident in the North York Toronto Health Partner’s OHT has access to a family doctor or nurse practitioner working in a publicly funded team by 2029. This individual will be a dynamic leader who is passionate about primary care and building systems, an excellent communicator and coach, thrives in a dynamic and evolving environment, skilled at navigating ambiguity, and can create the structure necessary for successful planning and implementation during a time of significant system change. They will need to build partnerships among the many health and social care partners in the community and act as a point of contact to ensure programs and services meet identified needs. Responsibilities:
  • Advancing the primary care access and attachment strategy of North York.
  • Being the lead and ensuring the implementation of initiatives identified by NYTHP based on agreed to deliverables and outcomes.
  • Advancing system development through spread and scale, including promotion through education, research, and evaluation.
  • Promote leading patient delivery models, and implement best practices related to primary care, including advancing the use of digital tools to improve experience and efficiency.
  • Act as a point of contact for the committees and initiatives you are assigned to lead.
  • Maintain and/or develop relationships with external networks, related professional organizations, and other related service providers to support shared goals.
  • Perform other related duties as assigned by the FHC’s Director, Community Health.

Qualifications:
  • Masters level preparation in Health or Business Management or equivalent preferred
  • Minimum three years of supervisory/managerial experience
  • Minimum seven years of experience working in primary care, health systems or related similar industries
  • Ability to design, deliver and evaluate health system programs and services
  • Clinical leadership experience is an asset
