Job Description

Reporting to the Manager, Coordinated Access, the incumbent will work as a member of the professional team to provide the following:

  • Triaging of new referrals to determine appropriate next referral management steps
  • Standardized referral screening (primarily via telephone, occasionally via videoconference) including level of care assessment through a utilization management tool (the LOCUS) for addictions and mental health
  • Matching clients to appropriate resources utilizing LOCUS results, service criteria, established standards, and clinical judgment
  • Follow-up with individuals and referrers as required to ensure timely and appropriate system navigation
  • Contribute to quality improvement initiatives to improve the services one-Link provides
  • Other duties as assigned

Qualifications
  • Bachelor’s degree from a social service/ healthcare program
  • Regulated Healthcare Professional (Registered Nurse, Social Work, Occupational Therapy) preferred
  • Member in good standing with respective regulatory body
  • Minimum of 5 years recent experience providing clinical services within the addictions and mental health sector
  • Broad knowledge within mental health, concurrent disorders, and addictions including withdrawal management
  • Exceptional time management skills and attention to detail, including proven ability to follow complex processes, comfort with rapid change, and ease with providing a balance of efficiency and client-centeredness within brief clinical encounters
  • Exceptional customer service skills, including timely response to inquiries and appropriate follow-up, following a commitment to exemplary patient experiences always
  • Excellent interpersonal and communication skills with demonstrated ability to build positive relationships with internal and external stakeholders
  • Demonstrated ability to work independently and as part of a multi-disciplinary team in a fast paced/high volume environment
  • Advanced proficiency with Microsoft Office software applications
  • Bilingual (French) and/or demonstrated fluency in a second language other than English is an asset
  • Knowledge of the various service systems within the Mississauga Halton Region
  • Knowledge of Meditech Expanse and/or Novari eReferral software an asset

Additional Information

Hours: Monday - Saturday - 7.5 hour shifts - 0800-1600, 1200-2000 (Hours subject to change based on operational requirements)

Salary: $39.512 to $51.763
