JOB SUMMARY:

Under the manager of the regional chronic diseases, the incumbent is responsible for initiating the stages of the nursing process and managing episodes of care within a helping relationship to promote diabetes self management and encourage the prevention of complications associated with uncontrolled diabetes. The diabetes educator nurse contributes to the delivery of exceptional quality clinical care while respecting the standards, philosophy and objectives of the nursing practice adopted at the diabetes clinic including people with diabetes who have an insulin pump as a management tool. She acts as a role model, teacher and resource person with patients, primary health care providers, direct care providers, and other community groups regarding best practices in diabetes management. It provides diabetes education based on best practices based on clinical practice guidelines for the prevention and treatment of diabetes in Canada from the Canadian Diabetes Association. This includes a nutrition-focused approach and the ability to educate patients on various aspects such as carbohydrate counting in relation to blood glucose management.

REQUIREMENTS:

  • Member in good standing with the Nurses Association of New Brunswick;
  • Current BLS training;
  • Bachelor’s degree in Nursing;
  • Minimum of five years of clinical experience with a clientele suffering from complex chronic conditions, including two years with diabetic patients (preferably within the past seven years);
  • Successful completion of required certification programs for medical delegated functions and advanced nursing skills specific to the unit as per set time frame ex: insulin pump;
  • Diabetes education certificate as per set time frame after obtaining the position;
  • Certificate in the NB insulin adjustment accreditation program as per set time frame after obtaining the position;
  • Accreditation by the New Brunswick insulin adjustment accreditation program;
  • Effective communication and education skills;
  • Good time management and priority setting skills;
  • Problem-solving skills;
  • Working knowledge of Microsoft Word, Excel and PowerPoint;
  • Willingness to use own vehicle and travel within the zone;
  • Possession of a valid driver’s licence and good driving record;
  • Ability to work independently and as a team member;
  • Physical ability to perform assigned duties;
  • Good previous work history (performance and attendance);
  • Adherence to professional ethics principles, to the Network management philosophy and organizational values;
  • Compliance with the rules of confidentiality established by the Vitalité Health Network.
