As a member of Peter Munk Cardiac Centre team, the Medly Clinical Coordinator coordinates and participates in the management of cardiovascular patients from northern Ontario, notably communities within Sioux Lookout First Nations Health Authority (SLFNHA) and Weeneebayko Area Health Authority (WAHA). The Medly Clinical Coordinator (RN) manages patients in these northern communities using the the Heart Function remote patient monitoring program Medly™. The role works alongside the UHN team dedicated to the SLFNHA/WAHA partnerships and liaises with PMCC triage offices, Inpatient and critical care units; referring physicians and surgeons and cath lab staff. Applies critical thinking, excellent communication skills and culturally appropriate approaches while using current knowledge of policies, procedures and processes to enhance therapeutic relationships with patients and Essential Care Partners (ECPs).
