Position Summary:
In accordance with established standards, professional requirements and as an integral member of the multidisciplinary health care team the Discharge optimization specialist provides clinical leadership and guidance for frontline teams and leaders in complex discharge practice while fostering interprofessional collaborations with teams, sites and community partners. The primary responsibilities includes ensuring discharge practices are in compliance with Ontario Health directives, organizational priorities by supporting the right patient in the right place at the right time through the continuum of care and across UHN sites; identifying educational opportunities and collaborating with other stakeholders to build capacity and ensure compliance with best practices; acting as a clinical resource for the interprofessional teams; working collaborative with GIM transitional care coordinators to ensure effective and efficient discharge process are in place; participating in relevant research activities projects etc demonstrating a commitment to professional lifelong learning; performing reflective practice; performing other duties consistent with the job classification as required.
