* Minimum completion of the third (3) year of a Bachelor Degree Program in Health Sciences, Nursing and or Health Project Management
* Province of Manitoba Class 5 Drivers License, or equivalent from province of residence.
* Proficiency with Microsoft programs (Outlook, Word, Excel, Teams and PowerPoint), as well as Internet applications and other Information
* Demonstrated organizational skills, and the ability to work independently
* Demonstrated problem solving and decision-making skills
* Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment
* Demonstrated knowledge and competence of skills and concepts related to the position

POSITION SUMMARY:
Collaborating with professionals in the Chronic Disease Education Program this position will conduct literature searches, organize findings, support planning and development of clinical decision tools related to chronic disease management team practice and decision tool use. The work undertaken as above will facilitate creation of a needed pillar for safe and successful team based primary care management of chronic conditions such as diabetes, COPD, and heart failure.
RESPONSIBILITIES:
Overview:
- Develop an understanding of regulations within the regulated Health Professions Act related to reserved acts.
- Develop an understanding of the Chronic Care Model, its key principles and delivery system design intended to facilitate the practice of inter-professional care and promotion of self-management.
- Working in consultation with the manager and team, plays a key role as it relates to program model planning and clinical decision tool development.
- Independently organizes and plans project timeline for completing established goals.
- Participates in team meetings and working group meetings.
- Other duties as assigned.
