Job Description: The incumbent will provide case management, assessment and counseling services to adults who are experiencing changes in their quality of life due to the deterioration of their mental health. Working collaboratively with a number of MHAS providers and the community, the primary responsibility of the incumbent will be to provide assertive outreach case management services in a variety of community settings to a wide range of adult clients. The incumbent will work as part of a team focused on providing psychosocial case management supports to persons in need of assistance with understanding their illness, recovery planning, housing, ongoing engagement and assessment, coordination of services and supports and managing crisis as relevant. Depending on client demand the incumbent may be assigned to provide counseling supports to other MHAS Teams as required. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement

Experience
- 1 year experience in the area of mental health and addiction services
- 1 year of supervised clinical counselling
- Experience working with mental health and addictions clients
- Experience and training in the following: Solution-Focused Counseling
- Experience and training in the following: Motivational Interviewing
- Experience and training in the following: Cognitive Behavioral Therapy
Education
- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license

- Demonstrated knowledge and ability for competent case management practice
