Job Description: Primary responsibility for Chronic Disease Management programs in the area of diabetes, assisting with development/coordination/ conduct of inpatient and outpatient programs and clinics. Utilize established criteria to assess and plan each client's selfmanagementcare needs, providing appropriate intervention to ensure timely access to care. Provide efficient and effective individual self-management education/counseling for patients/families/caregivers in all aspect of diabetes disease management care and treatment. This will include liaison with families, physicians, dietitians, staff nurses, and community agencies. Coordinate and facilitate group education classes for outpatients in a variety of settings. Promote continuing health professional involvement in patient education through effective communication, development of resource materials and provision of in-service education. Participate in departmental/research activities (program development, planning, quality improvement, and evaluation). Implement program/resource development related to needs assessment, planning, program design, implementation and evaluation.

Experience
3 years related clinical nursing experience with patients and families with diabetes disease management required. 1 year experience in adult education and group facilitation related to the area of cardiovascular and diabetes disease management required. 1 year experience in program/resource development related to needs assessment, planning, program design, implementation and evaluation, teaching/counselling, facilitation skills and leading group activities required. Experience working with complex patients and families in an outpatient setting with multiple co-morbidities including cardiovascular disease, diabetes, sleep apnea, COPD
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Master of Nursing, Adult or Continuing Education is an asset
- Basic Life Support Certificate (BLS)
