Job Description: The Primary Health Registered Nurse serves as a point of access to an integrated health care team, participates in collaboration with the patient, in identifying health needs and coordinates resources toward meeting those needs. The incumbent will be required to work field hours and may be required to work into the evening. Working in a multidisciplinary environment including, but not limited to, Primary Health Care Team members: Family Physicians, Mental Health Counselor, Home Care, Therapy services, Public Health, Dietitians, Healthy and Home, Psychiatrists, Medical Office Assistants and other team members as this position requires networking with other professionals and agencies within the community.

- Bachelor of Science in Nursing (BScN)

- Equivalent combination of education and experience is acceptable
