As a member of the multidisciplinary healthcare team, the Social Worker will apply their clinical knowledge and skills to provide comprehensive care, and follow-up to children ages 0-18.


Qualifications
  • Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker.

  • Master's Degree in Clinical Social Work from an accredited School of Social Work

  • 3-5 years clinical experience (within the last 6 years) with children and families dealing with acute and chronic medical illness, death and dying, bereavement issues and chronic progressive illness

  • Hospital experience is preferred

  • Excellent knowledge of community support and placement services

  • Knowledge of Healthcare Consent Act, Substitute Decisions Act and Child and Family Services Act.

  • Proven ability to work as a member of interdisciplinary healthcare teams.

  • Demonstrated skills in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills

  • Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem solve effectively

  • Demonstrated commitment to ongoing learning

  • Strong interpersonal, communication, teamwork, consultation and teaching skills
