Peer Support Provider

As part of the Outpatient Complex Care Team the Peer Support Provider will play a critical role in supporting patients and their families who are navigating the challenges of serious illness diagnosis, and compiling resources to support families throughout their unique healthcare journey. This role is ideal for someone with lived experience who can provide empathetic listening, non-judgmental support, system navigation and connections to community resources.

This role works closely with Outpatient Complex Care Social Worker, team and healthcare providers as part of day to day operations, as well as community organizations to help families feel supported, understood, and connected throughout their healthcare journey. Peer support will be provided in person and virtually to best meet the family needs. Peer Support is attached to the outpatient clinic structure however support can also be provided in an inpatient unit or patient’s home.


Qualifications

1.    High school diploma required

2.    University of college degree or diploma in social sciences or related field an asset

3.    Certificate in Peer Support Training from a recognized training provider or relevant work experience with demonstrated ability and understanding of core essentials of peer support provision preferred.

4.    Access to a vehicle as needed

5.    Ability to go to community sites to conduct home visits

6.    Experience with equity deserving communities and/or combination of lived experience

7.    Experience working with medically complex youth and inter-professional teams.

8.    Proficiency in additional languages is strongly preferred

9.    Proficiency in using electronics (computer, cell phone) and electronic platforms in order to provide virtual support to families
