 Reporting to the Clinical Leader of the HHS Virtual Nursing-Surgical Transitions program
• Identifying and confirming Patient eligibility for monitoring virtual care monitoring models with attending Physician and Registered Staff
• Ambassador of the virtual surgical transitions program, and visiting patients & families in hospital to provide program information
• Registering patients in EPIC and Remote Patient Monitoring platform when new referrals are received
• Booking patients for scheduled and unscheduled video visits with RPNs, RNs, and MDs and notifying patients of their upcoming virtual care appointments and how to prepare for the appointment
• The Business Clerk would be responsible to schedule all virtual visits appointments; other responsibilities include: coordinating all internal and external communication, communication with patients regarding rescheduling of virtual visits bookings
• Providing Patient with the device kit, Patient information package, Copy of signed Patient consent form, and shipping labels
• Managing in-coming and outgoing referrals, fax and other forms
• Managing project logs and data collection for program reporting
• Device management, which may include, but may not be limited to: labelling the devices, preparing the kits, tracking the location of the delivered and returned devices, clean the returned devices, wipe the data from the returned devices, and coordinate device delivery to enrolled patients
• Managing device logs, shipping requirements and coordinating with shipping companies as required


Qualifications

1. Grade 12 or equivalent
2. Graduate of community college secretarial or office administration program or equivalent or equivalent office experience
3. Medical Terminology Certificate 1 and 2 or demonstrated knowledge equivalent to the Medical Terminology Certificate 1 and 2 which may be subject to testing
4. Keyboarding 40 w.p.m.
5. Knowledge and skill in using computer
6. Demonstrated effective communication (verbal and written), interpersonal, public relations and problem solving skills
7. Ability to set priorities and organize work accordingly
8. Interested in learning new virtual care models, platforms and workflows and adapt them in day to day work
9. Ability to deal with various hospital departments effectively and collaboratively
10. Ability to appreciate the confidential nature of the position
11. Ability to sit for long periods, repetitive reaching, repetitive bending 
12. Previous experience and expert skill in EPIC workflows, scheduling templates, lab referrals, fax, Outlook,  and Microsoft Excel
13. Tact and discretion dealing with patients and families
14. Ability to work under pressure, changing priorities meet deadlines and assist other in the completion of assigned tasks
