Job Description

The SCOPE Navigator serves as the central access point for primary care providers (PCPs) needing to connect patients to appropriate hospital, community, and specialist resources. As part of the Seamless Care Optimizing the Patient Experience (SCOPE) program, this role supports timely care navigation, improves system efficiency, strengthens care transitions, and contributes to reducing unnecessary emergency department visits and hospital readmissions. The SCOPE Navigator collaborates with interdisciplinary teams and external partners to optimize integrated patient care pathways.

Key Responsibilities:

  • Serve as a point of access for PCPs, connecting them to appropriate clinical, community, and specialist supports.
  • Assess the primary care provider’s (PCP’s) complex concerns and apply a high degree of clinical decision-making to ensure appropriate and timely system navigation and service access.
  • Triage and forward referrals to the appropriate SCOPE contact (e.g., Internist, Nurse Navigator, OHaH Care Coordinator).
  • Collect and document patient information for assessment and database entry, ensuring accurate referral processing.
  • Support care coordination by clarifying PCP concerns, facilitating transitions, and improving access to hospital and community resources.
  • Identify barriers to care and contribute to workflow improvements, referral optimization, and seamless care pathways.
  • Monitor, analyze & present program data, submitting reports for evaluation and continuous improvement.
  • Develop and foster linkages with external community organizations to enhance seamless, integrated patient care.

Qualifications

Education:

  • Current membership in good standing with a regulatory health professional body in Ontario
  • An undergraduate degree in a health-related profession required
  • Master’s Degree in a related health discipline preferred

Experience:

  • 3 -5 years of experience within acute care and/or primary/community care
  • Experience working within an Interprofessional team environment as well as independently and autonomously
  • Experience working with older patients with complex medical needs required

Skills:

  • Must be flexible and able to adapt to changing environments and travel outside of hospital
  • Excellent interpersonal and decision-making skills
  • Strong organizational skills
  • Ability to adapt to a changing environment and assume responsibility for own learning
  • Demonstrated leadership skills and ability
  • Computer literacy required - Database management skills preferred

Additional Information

Status: Temporary Full-Time (x1)

Location: Tri-Site, travel will be required

Hours: Monday - Friday, days, evenings, 0800-1700h (Hours subject to change based on operational requirements)

Salary: $46.011 to $56.425 per hour
