The role of the Registered Practical Nurse in the Geriatric Assessment Clinics is to support the provision of care and treatment of health conditions for the older adult. The Registered Practical Nurse will be responsible for acquiring updated patient health information and addressing any issues that may arise during visits. The Registered Practical Nurse will be responsible for completing assessments, triaging referrals, linking patients and families with community resources and services and providing education. The Registered Practical Nurse will work collaboratively with the team to deliver comprehensive patient care, to facilitate positive patient outcomes and improve the overall well-being of the individual.

- Current registration in good standing with the College of Nurses of Ontario
- Nursing experience in the Geriatric setting required
- Current BCLS Certification required at time of hire
- Pain and Symptom management training and knowledge an asset
- Recent nursing experience in acute medicine and/or palliative an asset
- Excellent verbal and written communication skills
- Computer proficiency
- Caring attitude with patients, families and co-workers
- Must be physically able to do the job that involves heavy lifting and intensive physical effort.
- Strong physical assessment skills is required (preference will be given to applicants that have taken the physical assessment course)
- Excellent critical thinking skills, with an ability to set priorities and work effectively in a fast-paced environment
- Ability to work effectively and collaboratively in a team environment
- Demonstrates commitment to continuing education.
- Satisfactory employment and attendance record.
Hours: Monday to Friday - Various rotating 7.5 hour shifts (hours subject to change based on operational requirements)
Salary: $36.08 to $39.02 per hour
