Job Description

Mississauga-Halton Central Intake Program is the single access point for referrals for diabetes education services in the Mississauga Halton region.

The Central Intake Clinician is accountable for managing the day-to-day clinical activities related to diabetes referral management. This includes:

  • Review incoming referrals to diabetes education programs and other related chronic disease services for clinical acuity, content completion and service request
  • Triage and route referrals to appropriate diabetes program following criteria and guidelines
  • Collaborate with internal and external stakeholders to ensure ongoing evaluation and streamlining of the referral process
  • Conducting data analysis of referral patterns to identify gaps services
  • Develop and maintain appropriate triaging and decision tools and updated policy and procedure manual to support the central intake process and operation
  • Developing methods to ensure integrated care coordination between the Central Intake Program, referring physicians and diabetes education programs
  • Assist with the adoption and utilization of the eReferral solution for diabetes referral management
  • Develop targeted quality improvement strategies in collaboration with specific stakeholders to implement ongoing process improvement and adoption of the Central Intake Program, including participation in regional planning activities

Qualifications
  • Registered Nurse or Registered Dietitian
  • Certified Diabetes Educator certification preferred
  • Minimum 3 years experience in direct diabetes education and management
  • Demonstrated ability in clinical assessment, critical thinking and problem-solving
  • Demonstrated skills in collaboration, organization and change management
  • Strong knowledge of regional diabetes education and other chronic disease services
  • Understanding of healthcare delivery processes, benchmarking, activity tracking
  • Understanding of electronic referral solution and impact on workflow preferred
  • Ability to work independently and set work priorities in collaboration with team members
  • Strong written and verbal communication skills and excellent customer service skills
  • Able to operate with confidence in a self directed manner, comfort with new and evolving processes, strong problem identification and resolution skill
  • Knowledge of other languages, including French is a strong asset
  • Must be physically able to do the job
  • Excellent employment and attendance record

Additional Information

Hours: Monday to Friday, 8am-4pm

Salary: $46.004 to $59.637 per hour
