SUMMARY OF THE POSITION
Reporting to the Regional Proximity Director Miyupimaatisiiun Community Pole-Waskaganish, the incumbent is responsible for ensuring the effective organization and integration of primary and extended care programs and services. Through strategic planning, implementation, and evaluation of frontline services, they work to deliver a high-quality client experience within the Eastmain Community Miyupimaatisiiun Center (CMC) relationship-based service delivery framework.

The incumbent collaborates closely with the community leadership and key partners to achieve shared objectives that support the Miyupimaatisiiun of the population. Additionally, they coordinate with regional services, including the Director(s) of Regional Facilities (Hospital, Birthing Homes, Healing Center, Elder’s Homes, Intermediate Resources, etc.), ensuring seamless care access for families.

The incumbent promotes just culture principles for continuous improvement to care while maintaining compliance with functional, legal, and professional responsibilities across Nursing, Psycho-Social, Medical, and Allied Health disciplines.

They are responsible for managing human, material, financial, and information resources across multiple services and programs within the integrated CMC framework. Their work directly supports the Strategic Regional Plan (SRP) by promoting Miyupimaatisiiun, family-centered care, and a seamless healthcare journey for clients. This holistic approach ensures coordinated, client-focused services that enhance overall well-being.

This position is being posted as part of the ongoing transition toward the NISK model of care. While the model will be implemented gradually, this role will support the preparation and alignment of services as the transition unfolds.

SPECIFIC FUNCTIONS
Strategic Responsibilities
• Ensures the management of operational access, leadership, accountability, planning, organization, coordination and reporting, of the CMC Integrated Primary and Extended Care Services which includes the Multi-Service Day Center, Cree Home and Community Care Program, Pharmaceutical Dispensaries, Allied Health, Psychosocial and Current Emergency Acute services.
• Oversees the implementation, supervision, performance, quality, and reporting of integrated primary and extended frontline services, programs, and interventions within the community. Ensures timely access to resources while maintaining adequate support for daily operations.
• Contributes to the development and ensures the implementation of initiatives aligned with the Miyupimaatisiiun direction mandate, including MSSS programs, pilot projects, and special initiatives, to enhance the health and well-being of the Eeyou/Eenou Istchee.
• Collaborates in the preparation of policies and procedures pertaining to primary and extended care resources management within his/her community.
• Contributes to the planning, development and evaluation of the care coordination for regional and specialized services to ensure a wholistic approach to the continuity of primary care services.
• Ensures financial reporting to demonstrate accountability to funding sources in link with key performance indicators within his/her community.

Delivery of Primary and Extended Clinical Care Services and Programs Responsibilities
• Participates in the planning, organization and care coordination of services.
• Incorporates approved policies, procedures, protocols and tools to support the delivery of quality clinical programs and services within the different facilities under the CMC operations.
• Ensures delivery of the programs and services consistent with the goals, values, strategic objectives, of the Strategic Regional Plan (SRP) of the Cree Board of Health and Social Services of James Bay (CBHSSJB) and related legislation and regulations, protocols, best-practices, tools and measures for quality assurance, notably the organization’s commitment to culturally safe practice.
• Plans, organizes, supervises, and directs all aspects of operations of the facilities, including service elements and use of resources.
• Ensures the establishment and maintenance of effective team functioning by providing direction to CBHSSJB staff under their line-management around specific and shared objectives which are guided by the strategic objectives and values of the organization. This translates into organizing and leading management meetings and contributing to individual team meetings.
• Ensures the implementation of the Clinical Coordination and Integration Committee (CCIC) with representation from various professional and paraprofessional disciplines.
• As a local healthcare leader, he/she collaborates closely with the internal emergency preparedness team while in support of the community Emergency Management Committee response team when required.
• Ensures presence at public forums to increase health literacy, feedback on services, and foster collaboration with the population for integrated primary care services.
• Ensures the organizational process of reporting incidents, accidents, sentinel events and near misses related to clinical programs, services and activities of professionals is respected to support the quality and risk management system for client safety.
• Ensures that programs and services uphold respect and individual autonomy, addressing clients' bio-psycho-social needs while honoring their cultural and spiritual values. Maintains alignment with a collaborative and integrated model of care rooted in Cree cultural principles.
• Ensures that the best-practices for healthcare management are implemented, and ensures the quality of services offered maintains a perspective of continuous improvement and evaluation, in support of the organization’s mission, vision and values.

Proximity Services and Key Partnerships
• Collaborates with the Chief and Council, and community partners to establish, organize, and achieve local Health and Social priorities. This is achieved through supporting local planning structures (e.g., the Miyupimaatisiiun Committee).
• Facilitates the coordination and programming of clinical services in collaboration with the Pimuhteheu, Public Health, Nishiiyuu, and Wiichihiituwin departments to enhance local frontline service delivery. This approach fosters a seamless, integrated care, ensuring interdisciplinary collaboration that strengthens community health outcomes.
• Ensures access to established corridors of services for secondary, tertiary, specialized and super-specialized services outside of the region.
• Ensures the collaboration protocols with local and regional community service providers are followed to enhance service delivery and minimize risks (i.e. Youth Protection and CMC Collaboration Protocol).
• Ensures the effective implementation of the program framework across all client sectors within the community, addressing diverse health and social needs. This includes fostering collaboration, optimizing resource allocation, and aligning services with community-specific challenges to provide comprehensive and responsive support.

Administration and Reporting Responsibilities
• Ensures human, financial, material, and information resources management under his/her jurisdiction.


REQUIREMENTS
Education & Experience
• Bachelor degree in administration or Bachelor degree in health care or social services with a certificate in administration;
• A candidate of the Cree Succession Program - Management Talent Program or First nations Health Manager Association program will be considered;
• Ability to apply Eeyou (Cree) culture, values, traditions and teachings into programs and services;
• Four (4) years’ experience in supervision or administration in a social service or rehabilitation environment.

Knowledge and Abilities
• Good knowledge of the MSSS and Quebec Healthcare programs, laws, regulations, the RUIS Network;
• Good knowledge of population health, program and service management, including the development of policies and quality assurance systems;
• Good record at an appropriate level of healthcare or programming management;
• A good record in human, financial, and information resources management is an asset;
• Ability to apply Eeyou/Eenou (Cree) culture, values, traditions and teachings into programs and services;
• Ability to both manage the planning, development and implementation of new programs, and maintain quality assurance of existing programs;
• Excellent interpersonal communication, leadership and respectful teamwork skills.


LANGUAGE
• Fluent in Cree and English;
• Fluency in French is an asset.

OTHER
• Willing to travel.
