The Addictions Program is seeking two (2) full time, contract (15 months) Social Worker 2 to work in Concurrent Outpatient Medical Psychosocial Addiction Support Service (COMPASS) to support specialized initiatives, including HART Hub Collaborations. Reporting to the Manager, the incumbent will provide direct clinical care for adults who are experiencing addiction or concurrent disorder through a team-based, interprofessional approach to care.
Key responsibilities include delivering outpatient psychosocial treatment, primarily through structured/manualized and unstructured group-based interventions, within a phased model of care, from early engagement to skills-based treatment. The role also includes the provision of limited individual treatment. Additional duties include conducting assessments and referrals for new clients, engaging with families and community supports, and providing system navigation support. The ideal candidate will demonstrate a trauma-informed and recovery-oriented approach, and possess the ability to work effectively with diverse populations. Strong knowledge of community resources and experience working with families/significant others are essential. The role requires excellent interpersonal and networking skills to coordinate client flow across services, liaise with community partners, and support the education of clients, families, and external providers. Other related duties may be assigned as required.
The successful candidates will also contribute to continuous quality improvement initiatives, support research and education activities, engage in program development and capacity-building efforts, and foster strong relationships with service providers and community partners. The role includes the supervision of Master of Social Work (MSW) students, collaboration with faculties of social work on special projects, and contribution to best practice development within the field. Teaching and knowledge exchange with other professionals is also expected.
The incumbents will contribute to a healthy, inclusive workplace that embraces diversity, fosters teamwork, and adheres to all applicable regulatory and legislative requirements. This position is located at 1001 Queen Street West and HART Hub partner sites across Toronto, evening hours may be required.
The successful candidate will have a Masters of Social Work degree and be registered in good standing with the Ontario College of Social Workers and Social Service Workers, supported by three (3) years preferred of clinical experience with mental health/addiction setting. You must be eligible for an adjunct faculty appointment with faculties of Social Work that have affiliation agreements with CAMH in order to supervise social work students. Demonstrated experience in facilitating both individual and group treatment, discharge planning, conducting assessments, providing psychosocial support and psycho-education is required. Preference will be given to those who possess knowledge of motivational interviewing, cognitive behavior therapy (CBT), dialectical behaviour therapy (DBT) and the ability to apply principles of risk management. Candidates with sound knowledge and understanding of social work practice with the addictions/concurrent disorder client population are preferred. Experience working with individuals from diverse ethno-cultural backgrounds is required. You will also possess excellent organization, communication and interpersonal skills and the ability to work effectively as a team member. Experience working with individuals from diverse ethno-cultural backgrounds is also required. Bilingualism (French/English) is an asset.
