Description
Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. For more information Assisted Living Alberta. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The RSW-SW II independently provides generalist social work services and manages a complex caseload of Alternate Level of Care (ALC) clients. The role focuses on addressing social, legal, and financial barriers to support smooth transitions to and within Continuing Care. Working collaboratively with health care teams, government partners, contracted service providers, and community agencies, the RSW-SW II coordinates care, advocates for clients, and contributes to quality improvement initiatives. Key Responsibilities: Conducts assessments to identify discharge barriers and eligibility for programs and resources. Develops comprehensive care plans considering psychosocial, legal, and systemic factors. Coordinates services and supports integrated, client-centered transitions. Provides consultation on complex discharge planning and financial resource optimization. Applies relevant legislation to guide ethical and lawful practice. Mediates conflict between clients, families, and care teams. Builds partnerships to enhance access to services and monitors caseload activity.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP). Experience in acute care, transition, or continuing care settings. Strong knowledge of discharge planning, financial supports, and housing systems (PDD, Recovery Alberta, Continuing Care, Supportive Living, Market Housing). Excellent communication, critical thinking, and teamwork skills. Demonstrated professionalism, ethics, and understanding of decision-making legislation.
