Are you a skilled case manager with a passion for leveraging technology to support client care? If so, the Central Zone Continuing Care Access Centre’s virtual team may be the perfect fit for you! The successful candidate may work from any Alberta Health Services site within Central Zone, with workspace arrangements negotiable. Reporting to the Continuing Care Access Centre Manager, the Registered Nurse - Intake Nurse will become part of a dedicated team of nurses specializing in intake, screening, and client navigation. In this role, you will serve as the first point of contact for individuals seeking Home and Community Care or Continuing Care Home services. You will assess initial needs, determine appropriate pathways, and connect clients to both internal and external resources. Working within multiple computer applications, you will complete real‑time phone assessments, conduct initial contact assessments, and collaborate with key stakeholders as needed. Success in this position requires the ability to prioritize referrals from diverse sources, work efficiently and independently, and maintain a client‑centered approach while balancing a fast-paced workload.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Current registration as a Registered Nurse (RN) in good standing, with a minimum of two (2) years of clinical experience, preferably within Home Care, Continuing Care, Transition Services, or Home and Community Care settings. Demonstrated knowledge of home care operations, continuing care systems, and community-based resources. Proven excellence in both written and verbal communication, with the ability to engage effectively with clients, families, and interdisciplinary team members. Highly self-motivated, adaptable, and flexible, with the ability to thrive in a fast paced, dynamic environment. Strong team player with the ability to collaborate, problem-solve, and contribute positively to team culture. Demonstrated ability to navigate and work efficiently across multiple computer programs while conducting client assessments by phone. Strong working knowledge of Connect Care and Microsoft Office applications. InterRAI competency and completion of AHS Case Management training (or willingness to obtain).
