Description
Are you looking for an exciting opportunity to showcase your experience in Seniors Health? Join an amazing team of professionals that are responsible for the day-to-day operations and activities related to client's access to Continuing Care Homes in the Central Zone. The Continuing Care Home Coordinator uses a collaborative approach to review all applications for Continuing Care Homes, as well as manage the Central Zone wait list according to guidelines and policies in place. The coordinator will also act as a resource for facilities and community health professionals, revise priorities on a continuous basis and coordinate the placement of individuals in Continuing Care Homes in the Central Zone.
As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.
Active or eligible for registration with applicable professional association or college.
Current registration as a Registered Nurse (RN) in good standing, with a minimum of two (2) years of clinical experience, preferably within Home Care, Continuing Care, Transition Services, or Home and Community Care settings. Demonstrated knowledge of home care operations, continuing care systems, and community-based resources. Proven excellence in both written and verbal communication, with the ability to engage effectively with clients, families, and interdisciplinary team members. Highly self-motivated, adaptable, and flexible, with the ability to thrive in a fast paced, dynamic environment. Strong team player with the ability to collaborate, problem-solve, and contribute positively to team culture. Demonstrated ability to navigate and work efficiently across multiple computer programs while conducting client assessments by phone. Strong working knowledge of Connect Care and Microsoft Office applications. InterRAI competency and completion of AHS Case Management training (or willingness to obtain).
