Wetaskiwin Home Care is seeking an experienced Registered Nurse for its Home Care team. Reporting to the Care Manager, Wetaskiwin Home Care in the role of Case Manager, you will be responsible for coordinating and providing quality care to clients in the community. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning. Through the development of the care plan you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates and be on call. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Must be able to work in different environments across the program, from Community / Client homes, Independent Living Sites and Designated Supported Living (DSL3/4/4D). Effective time management and prioritization skills are required. Must be able to work independently and within a team environment. Ability to problem solve and demonstrate critical thinking skills is required. Conflict resolution and strong communication skills are required. Demonstrated ability to provide comprehensive assessments to individuals within their physical and social environments is required. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Knowledge and ability to use computer applications such as Meditech, Connect Care, Microsoft Office, Excel, and Outlook is required. Work tasks and charting is computer based so there are extensive repetitive motion tasks. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to participate in hands on care of patients (i.e., transfers, bariatric clients, bending, Reaching, twisting, and walking distances). Must be able to lift and carry 10-15 kg regularly if needed.

A minimum of 1 year community based nursing experience including geriatric experience (obtained within the last three years) is desired.
