As a Clinical Coordinator with Vancouver Coastal Health you will:
- Provide first line supervision, coordinates the day to day operations of the designated Clinic or Program and ensures that operations of the designated Clinic or Program comply with the applicable Health Canada requirements.
- In collaboration with all health care team members, coordinate client flow activities by reviewing clinic processes and developing protocols for improving client flow.
- Provide clinical direction and coordination of clients and assists the Manager with a variety of administrative activities to ensure efficient and optimal use of client care resources.
- Liaise with designated team members/staff and supports interdisciplinary practice, quality improvement initiatives and supports interdisciplinary integration of services.
ABOUT URGENT AND PRIMARY CARE CENTRES
Urgent and Primary Care Centres (UPCC) will provide care for people with unexpected, non-life-threatening conditions who need medical attention within 12-24 hours but who don't require a visit to the emergency department - this includes sprains, cuts and wounds, an infection, mild asthma attack and less serious child illness and injury. The multidisciplinary health care team will be staffed by general practitioners, nurse practitioners, registered nurses and social workers.
UPCCs are intended to be an additional service in the community, working in concert with all touch points for care to provide appropriate urgent services to patients, when and where they need it. They do not replace your Emergency nor your Family Physician access rather enhance access to urgent primary care when your doctor is not available and where you do not need emergency level care.
Education & Experience
- Current practising registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent, related nursing experience in the community health nursing including one (1) year related leadership/supervisory experience, or an equivalent combination of training and experience.
Knowledge, Skills & Abilities
- Comprehensive knowledge of other health care disciplines and their role in client care.
- Demonstrated ability to adapt to the changing health system and an understanding the broad determinants of health.
- Broad knowledge of the local Community and regional initiatives that may impact patient care, resource access or practice guidelines in this local clinic setting.
- Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative and professional judgment.
- Ability to supervise.
- Demonstrated ability to apply decision making and analytical skills.
- Demonstrated ability to provide leadership and work direction to the interdisciplinary team.
- Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
- Demonstrated ability to communicate effectively, both verbally and in writing.
- Demonstrated ability to deal effectively with conflict situations.
- Demonstrated ability to develop and evaluate health education resources.
- Knowledge of research process and methodology.
- Demonstrated ability to critically read and utilize current health research in evidence-based practice.
- Demonstrated ability to identify community resources appropriate to client/family/community needs.
- Ability to operate related equipment including applicable software applications.
- Demonstrated ability to plan, organize and prioritize work.
- Demonstrated skill in CPR techniques.
- Physical ability to perform the duties of the position.
The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
