As a Palliative Approach to Care Coordinator you will:

  • Report to the Leader of the Regional Palliative Approach to Care Education (RPACE) program and assist with establishing organizational competencies including organizational awareness and promotion of a palliative approach to care, including goals of care conversations.
  • Develop and deliver education to build clinician confidence and competence for identifying clients who may benefit from a palliative approach, which includes having goals of care conversations and documenting this information in the appropriate location.
  • Consult and collaborate with educators, clinical programs and team members in the promotion of quality improvement, and coordination of activities to support the early integration of a palliative approach.
  • Explore and analyze service data with the goal of providing valid, reliable information for measuring, monitoring and reviewing of outcomes in evaluating performance.
  • Support and develop teams throughout the region to develop interdisciplinary workflows around carrying out and documenting goals of care conversations for those with serious illness.
  • Collaborate with other RPACE Coordinators and regional stakeholders to ensure consistency where appropriate and assist each Community of Care (CoC) and Providence Healthcare as required.
Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years’ recent, related nursing experience in palliative care or related specialty, including one year's related teaching experience and/or group facilitation or an equivalent combination of education, training and experience.
  • Completion of ‘Educator Pathway Program’ is preferred.
  • Valid Driver's License to operate a vehicle within B.C. as local area travel may require the use of a vehicle.

  • Broad knowledge of health care policies, procedures, regulations and legislative environment.
  • Demonstrated experience in the development and delivery of professional education.

  • Comprehensive knowledge of Trauma Informed Practice, Cultural Safety, Harm Reduction and Recovery Oriented Care.
  • Broad knowledge of behaviour management and ability to utilize non-violent crisis intervention strategies in a trauma informed manner.
  • Demonstrated knowledge of the impact of colonization on Indigenous peoples.
  • Broad knowledge of facilitation, mediation, and conflict resolution techniques and demonstrated analytical and problem solving skills.
  • Broad knowledge of quality improvement processes and methodology.
  • Broad knowledge of project management and change processes.
  • Demonstrated customer focus and ability to engage stakeholders.
  • Excellent verbal, written and presentation skills.
  • Demonstrated ability to lead effectively in a highly dynamic environment subject to continuous change.
  • Ability to work effectively under pressure and with changing priorities and deadlines.
  • Ability to collect and interpret data and provide recommendations on next steps.
  • Proficiency in the use of office software applications as effective tools to achieve results.
  • Knowledge of clinical practice related to treatment and care of the designated patient/resident population.
  • Knowledge of organization’s policies, procedures and standards of care.
  • Knowledge of other health care disciplines and their role in patient/resident care.
  • Knowledge of research methodology and processes.
  • Demonstrated clinical proficiency in the designated clinical area.
  • Demonstrated ability to teach, demonstrate, facilitate and lead.
