POSITION SUMMARY:

Reporting to the Director, UHN Connected Care, the Senior Project Manager is a key member of our leadership team that can listen, collaborate, connect the dots, and build trust in a team. They are able to think strategically, act practically and implement judiciously all while building commitment to a collective goal.

The Senior Project Manager position will focus initially on leading the planning and implementation of active remote care, and leading improvement and expansion in other areas of the Clinical Operations portfolio. In the longer term this role may also work in other parts of Connected Care.

Our work involves engagement, co-design and partnership with clinical programs, community partners and patient partners to improve patient transitions and improve care for patients, in the right place at the right time. This role will be responsible for managing multiple concurrent initiatives that are complex and span across the continuum of care locally at UHN, regionally and/or provincially. It also involves working with multiple organizations and engaging different levels of leadership across the healthcare sector. Critical to this role is the ability to apply a health system lens to bring multiple sectors together to design and implement an integrated system of care enabled by digital health.
