The Transitional Care Program (TCP) at UHN is a collaborative and innovative approach designed to help patients who no longer need acute care services, but often find themselves waiting for an alternate care facility, such as convalescent and long-term care. Patients and families have access to the specialized care needed in a restorative setting that allows for patient optimum functioning and quality of life. The TCP philosophy is all about delivering patient care in the most appropriate setting where patient privacy, dignity and safety are top priorities.
Position Summary
Reporting to the Director of Professional Practice (DPP), the Manager of Professional Practice (MPP) for the TCP will be covering clinical programs at all 3 TCP sites: 82 Buttonwood Avenue, Toronto (Buttonwood RCC); 37 Austin Terrace, Toronto (Hillcrest Reactivation Centre), and 265 Queen’s Drive, North York (Queen’s Estate). The ideal candidate will have a nursing background, will be a member of the UHN Collaborative Academic Practice (CAP) portfolio, as well as a TCP Clinical Leader. The successful applicant will support the development, implementation and maintenance of profession-specific and interprofessional practice standards at the program level and across UHN, as required. The MPP will also be required to participate in the TCP on-call process, on a rotating basis.
