Position Summary

The Clinical Coordinator is an advanced healthcare professional responsible for the clinical coordination and facilitation of complex patient care activities across multiple specialties and services. Leveraging expert clinical knowledge, critical thinking, and professional judgment, the Clinical Coordinator serves as a key liaison between patients, families, physicians, multidisciplinary care teams, external healthcare providers, and funding organizations. The role is accountable for navigating highly complex clinical situations, facilitating access to specialized care, supporting treatment planning and service delivery, and ensuring a seamless patient experience.
