Job Description:

Position Overview:
With growing demands for expert geriatrics services for our inpatient and Emergency Department populations, the Registered Nurse (RN) as a member of the Geriatric Medicine Consultation Service (GMCS) will provide consultation services for frail elderly patients with multiple comorbidities and complex medical regimes in hospital Working as a collaborative member of the interprofessional team, the RN (GMCS) functions in an expanded role, demonstrating a high level of autonomy and expert skill level in their nursing assessments. The RN will identify and navigate potential patients for GMCS consultative services, contribute to the comprehensive geriatric assessment, and support and facilitate recommended care plan changes with the care team. This role will include building geriatric knowledge with local and corporate care teams. They will provide expert knowledge and support to the inpatient and ED teams in the care and treatment of frail elderly patients and work in collaboration with Behavioural Support Ontario (BSO) and the psychiatric consultation liaison (CL) team as necessary. Hours will require flexibility and may include evening and/or weekend work.

Requirements:
• Current registration as a Registered Nurse (RN) with good standing with the College of Nurses of Ontario
• Completion of a Bachelor’s degree preferred
• 3 years of recent clinical work experience with frail older adults and the ability to demonstrate knowledge and assessment skills of the older adult.
• Certification in Geriatric Nursing from Canadian Nursing Association is preferred.
• Certified in Gentle Persuasive Approach (GPA). If the successful candidate does not have GPA certification, they will be required to complete GPA training within 2 months of hire.
• Knowledge and experience with geriatric-specific assessment tools such as, but not limited to, the Comprehensive Geriatric Assessment (CGA).
• Current Basic Life Saving (BLS) certification
• Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively
• Effective verbal and written communication skills
• Strong interpersonal, organizational and problem-solving skills
• Ability to work well within an inter-professional team and to foster effective working relationships
• Ability to exercise autonomy and initiative while observing strict confidentiality
• Proven good attendance record with the capability of maintaining this same standard
• Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
• Utilize the protective equipment provided and identify and report hazards
• Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
• Excellent work performance and attendance record

Asset:
• Specialty certification related to Geriatrics or Seniors Health an asset
• Experience with electronic documentation
• Membership in professional organizations preferred
• Ability to speak another language
• Gentle Persuasive Approach (GPA) instructor
