Position Overview: The Patient Care Coordinator is a Registered Nurse who provides support and expertise to patients and families living with serious, life limiting health conditions, and their care partners. The Patient Care Coordinator will liaise with inpatient teams, outpatient teams, community partners and other practitioners engaged in the persons circle of care. The goal of this role is to facilitate care planning, enhance pain and symptom management plans, and build capacity of teams to support patients who are engaged in a palliative approach to care. There may also be opportunity for community outreach, including home visits.The successful candidate will have an organization-wide focus in both inpatient and ambulatory clinic environments, providing consultation and supporting holistic palliative care planning and delivery. This role requires excellent critical thinking skills, in-depth knowledge of palliative care principles and advanced communication skills. There are three main areas of focus for this role:1. Identification of patients who would benefit from a palliative approach to care2. Education Work with care teams and patients/families to build knowledge and enhance development of appropriate palliative plans of care,3. Care transitions Participate as a member of the Palliative Rapid Response Team to support seamless transitions across care settings for inpatients being discharged home with a palliative approach to care. Support inpatients/families who have been discharged home for end of life care or ongoing palliative pain and symptom management in partnership with community-based members of the palliative care team. Interactions may be virtual or in-home. Responsibilities: Undergraduate Degree in Bachelor of Science in Nursing (BScN) (Masters preferred) Current registration with College of Nurses of Ontario (CNO) as a Registered Nurse Must have recent experience (within last 3 years) working with patients pursuing a palliative approach to care within a specialized program Demonstrated knowledge of pain and non-pain symptom management in cancer and non-cancer conditions Valid Ontario Driver's license and access to a vehicle Current BLS CPR certification Member of RNAO Canadian Nurses Association certification in Hospice Palliative Care Nursing [CHPCN(C)] Evidence of successful completion of the following palliative care coursework: Fundamentals of Hospice Palliative Care, DeSouza Pain and Symptom Management, CAPCE (Comprehensive Advanced Palliative Care Education) and LEAP (Learning Essential Approaches to Palliative Care) Core. Excellent knowledge and understanding of resources for grief and bereavement support for children and adults in GTA Well-rounded knowledge of homecare and community supports in Scarborough region Excellent physical assessment skills, critical thinking and professional practice judgment expertise Strong understanding of adult teaching/learning theories, tools and techniques Exceptional understanding and demonstrated experience in coaching and mentoring of clinical staff Demonstrates values consistent with SHNs Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
