Scarborough Ontario Health Team (SOHT) is one of the largest OHTs in the province and based in one of Canada’s most vibrant and diverse communities. We are a network of forty partner organizations with a rich history of collaboration providing health and community services across the continuum of care. Our OHT’s attributed population of almost 900,000 goes beyond the residents of Scarborough to include residents of neighboring communities that have made Scarborough their health care destination of choice.
Position Overview:
Are you passionate about driving transformative change in healthcare? The Scarborough Ontario Health Team (SOHT) is seeking a dynamic Project Manager to lead and support innovative, system-level initiatives that enhance primary care collaboration and integration across Scarborough.
This position plays a vital role in building capacity, empowering stakeholders, and using best practices in project management, design thinking, lean improvement, and change management. The Project Manager will support end-to-end project implementation, foster strong relationships with primary care partners, and help mobilize SOHT’s vision for an integrated, seamless, patient-centred health system.
Key Responsibilities:
Project Management & Implementation
- Lead project lifecycle activities: initiation, planning, execution, monitoring, and close-out
- Apply lean, experience-based design, change management, and facilitation methods
- Ensure timely delivery of project documentation, tools, and outcomes
Improvement Support
- Analyze workflows, identify improvement opportunities, and implement quality improvement tools (e.g. value stream mapping, 5S, FMEA)
- Support evaluation and continuous improvement aligned with SOHT standards
Portfolio & Stakeholder Engagement
- Support and update SOHT leadership and PCN Clinical Leads
- Promote and coordinate OH-funded programs (e.g. Online Appointment Booking, AI Scribe, FindHealth)
- Manage stakeholder relationships and respond to data/briefing requests
Tool Development & Maintenance
- Maintain and improve project and process tools (e.g. project charters, status reports, process maps)
- Ensure tools remain aligned with emerging best practices and SOHT priorities
Education & Coaching
- Deliver training and coaching on lean, project management, and quality improvement
- Support capacity building across SOHT and primary care teams
Requirements:
- Undergraduate degree required
- Master’s degree preferred (MBA, MHA); combination of education and experience may be considered
- PMP (Project Management Professional)
- Lean Six Sigma (Yellow, Green, or Black Belt)
- PROSCI Change Management or equivalent
- Regulated Health Professional designation preferred
- 5+ years of project management and/or quality improvement experience in healthcare or related field
- Proven track record of leading multi-stakeholder projects in complex environments
- Advanced skills in project management, process improvement, and stakeholder engagement
- Exceptional communication, critical thinking, and facilitation abilities
- Proficiency in MS Office (Word, Excel, PowerPoint, Outlook, Visio)
