Summary of Duties Sunnybrook Health Sciences Centre has an opportunity for a Speech-Language Pathologist to provide comprehensive swallowing and communication services to outpatients in the Geriatric Day Hospital. The successful candidate will work collaboratively with an interprofessional team to provide evidence informed best practice.   Responsibilities Include: Provide evidence-based assessment, management and counselling of patients with voice, speech, language, cognitive-communicative and swallowing disorders Act as a resource people to the interprofessional team, patients and families regarding swallowing and communication changes in older people Maintain patient records and workload measurement in accordance with facility and professional standards Lead individual and group-based models of clinical intervention and education Assist patient and families in setting realistic goals and objectives of effective patient management Participate in interprofessional and clinical education activities Participate in Specialized Geriatric Services, profession specific and hospital wide meetings, committees and planning as required Maintain and upgrade skills and knowledge through continuing education and seek opportunities for knowledge dissemination   Qualifications/Skills Current CASLPO registration. Previous experience working with older adults required. 1-2 years of clinical experience in an ambulatory / rehab setting preferred. Dysphagia experience (including videofluoroscopy assessment) preferred. Working knowledge of community resources, HCCSS, RGP, and specialized seniors programs. Experience facilitating group education for seniors preferred. Strong documentation skills and well versed in computer functions, typing speed 40-60wpm. Excellent communication and time-management skills and ability to work collaboratively as a member of interprofessional teams. Demonstrated commitment to person-centred care principles and best practices.   Evidence of good attendance, a satisfactory performance history and a discipline-free record required. Ability to demonstrate reflective practice.
