Summary of Duties Sunnybrook Health Sciences Centre has an opportunity for a Patient Service Partner to promote and maintain good public relations with patients, visitors and Hospital staff in the Nursing Units. The successful candidate will participate as a member of the team in a manner that reflects the Hospital philosophy and mission of service. Responsible for working with the professional team, the Patient Services Partner participates in the caring of patients by providing a variety of personal support services to the team and patients within the concept of Person-Centred Care. Picks up and delivers patient meal trays daily and refreshments as required. Assists patients in opening lids, pouring juices, etc. Transports patients to different areas of the hospital via wheelchair, stretchers or beds as necessary. Escorts patients to main entrance upon discharge from the units. Prepares room for admission of patients which includes making beds and ensuring all necessary equipment is at the bedside. Ordering/restocking and replenishing necessary supplies for the unit as required. Assists with routine patient care, including dressing and ambulation, under the direction of the professional team. Assists with transportation of patients’ belongings as required. Retrieves equipment and supplies from units as required. Cleans and tidies patient rooms and patient care devices between patients. Cleans and tidies kitchen and general environment. Escorts patients to external appointments as necessary. Performs other duties as required, under direction from Team Leader.   Qualifications/Skills Grade 12 education, PSW Certificate required. Experience working with the elderly a definite asset. Sunnybrook PSP certification required. Previous hospital experience an asset. Ability to work under pressure and cope with changing priorities, shifting from task to task with ease. Good interpersonal skills. Ability to read, write and speak English competently. Must have attended all corporate training, including WHMIS and Infection Control. Must be prepared and able to use all personal protective equipment. Ability to understand and carry out oral and written instructions. Ability to communicate with Nursing and other hospital staff. Liaises with patients and visitors on a regular basis demonstrating excellent customer service.
