Patient Care Manager I - K3E Acute Care Unit - Temporary Full-time 2024-8451

Summary of Duties: The patient care manager collaborates with team members to achieve operational and patient focused care objectives within the Integrated Community Program. The unit is a 32 bed unit (16 beds under General Internal Medicine Hospitalists, 16 beds under Family Practice Hospitalists). Patients admitted to K3EA come from any program across the hospital whose needs can be managed on the unit. The role includes creating and maintaining a supportive environment for patients and staff, providing leadership, facilitating staff and team development, managing budgets and material resources, and facilitating quality improvement   The PCM is also accountable for ensuring that care requirements are met, and that patient care issues, problems and concerns identified by patients and their families are addressed.   Qualifications/Skills • A Master’s Degree in Clinical/Health Sciences or Management preferred. Bachelor’s Degree required • A OCNO Certificate of Competence • Minimum three years acute care clinical experience • Recent relevant management experience • Demonstrated commitment to excellence and patient focused care, including shared governance and interprofessional care teams • Demonstrated excellent change management and project management skills • Demonstrated coaching and empowering style of management • Demonstrated a commitment to academic excellence and innovation • Superior leadership, communication, team building and decision making skills • Ability to manage effectively in a fast-paced, constantly evolving environment
