The Complex Out-Patient Antibiotic Treatment (COPAT) Clinic is an innovative Nurse Practitioner led clinic launching at Sunnybrook Health Sciences Centre to offload occupancy pressures in the Emergency Department (ED) and inpatient wards, by providing safe, effective and efficient outpatient care for patients on oral or intravenous antibiotic treatment for common and serious infectious diseases under the supervision of Sunnybrook Infectious Diseases division. Patients seen in the clinic will be Infectious Diseases patients referred from the SCOPE program, patients discharged from the wards, as well as those requiring urgent follow-up from the Emergency Department.   The Nurse Practitioner (NP) is an advanced practice role designed to contribute to the quality of patient care through practice, research and education.  The current emphasis of this role is direct patient care, including the daily clinical management of a defined group of Infectious Diseases patients in both outpatient and inpatient settings. Collaborates with Infectious Diseases physicians, Nurse Practitioners, Patient Care Managers, all members of the inter-professional clinical team, NP Professional Practice Leader as well as those of the various consult services and Nursing to enhance and improve nursing practice.    As a Nurse Practitioner leading the COPAT Clinic the NP will review patients with Infectious Diseases physicians, see the rapid new assessment patients from Emergency Department/inpatients and outpatient referrals from SCOPE program, as well as existing COPAT patients with urgent complications requiring in person assessment. The NP will manage complex wounds and administer first dose antimicrobials and provide required management of central venous access devices (e.g. PICC lines, ports). The NP will follow-up with all COPAT patients 1 week post-discharge, coordinate, check and manage COPAT bloodwork, order and complete RM&R for LHIN referrals and prescriptions and monitor NP.COPAT@sunnybrook.ca email to respond to patients.   Qualifications/Skills   •    RN (EC) license, current College of Nurses of Ontario Certificate of Competence in good standing •    Master’s Degree with Nurse Practitioner (NP) Adult or Primary Care Certificate required. •    Direct patient care experience required •    Advanced physical assessment and health assessment skills •    Strong organizational skills •    Patient-Focused Care approach with an understanding and appreciation of infectious diseases •    Venipuncture, insertion of IV catheters and management of PICC lines •    Excellent interpersonal, communication and team building skills •    Current B.C.L.S. •    Membership in R.N.A.O. •    Previous experience as a Clinical Nurse Specialist/Nurse Practitioner an asset •    Program planning and evaluation skills •    Demonstrated research abilities •    Demonstrated professional leadership and ability to be innovative and creative •    Experience precepting undergraduate, graduate, and post-graduate students, clinical fellows in their experience, and evaluating their performance.
