Dietitian- High Risk Postpartum- Temporary Part-time 2025-13643

Summary of Duties

The main functions of the Dietitian in the high risk clinics include, but are not limited to:

  • Reports to the Patient Care Manager
  • Provides antepartum/postpartum care to patients experiencing a complicated pregnancy
  • Conducts patient assessment, intervention, education, patient advocacy & labour and delivery planning
  • Provides care in the context of the interprofessional team and strives to give excellent care throughout the patient’s hospital experience
  • Collaborates with other members of the health care team in those activities provide excellent patient/family-centred health care
  • Maintains all core certifications as a requirement of employment
  • Provides support to patients with pregnancy complications with a focus on patients with diabetes (GDM, Types 1 & 2)
  • Provides nutritional counseling and support for patients with pregnancy complications with a specialized focus on women with diabetes (GDM, Types 1 & 2)
  • Provides consultative support for patients on W&B obstetrical inpatient units (High Risk Obstetrics, Maternal Newborn, and Birthing Unit).
  • The successful candidate will participate in the advancement of patient care through evidence-based practice Collaborates with other members of the health care team in those activities which facilitate the operation of the clinic and in providing excellent patient/family-centred health care.
  • Supports the philosophy of family-centred maternity care & the Mission, Vision & Values of Sunnybrook Health Sciences Centre.

 

Qualifications/Skills

  • Member of the College of Dietitians of Ontario (CDO) required
  • CDE (Certified Diabetes Educator) certification required
  • Recent, related experience with an obstetrical population required
  • Proficiency in nutritional assessment and recent experience working with perinatal patients with diabetes required
  • Experience in providing patient/family education and counselling required
  • Demonstrated initiative, teamwork and ability to effectively prioritize work required
  • Demonstrated effective, communication, collaboration, mediation skills required
  • Demonstrated application of evidence-based practice required
  • Demonstrated commitment to ongoing professional development required
  • Demonstrated excellent verbal and written communication & interpersonal skills are essential.
  • Experienced computer skills including IntelliSpace Perinatal, Sunnycare, Acurro required
  • Demonstrated interest in personal professional development
  • Demonstrated experience in conflict management
  • Excellent time management and organizational skills
  • Demonstrated team player with the proven ability to set priorities of care
  • Demonstrated ability to collaborate with members of the interprofessional team
  • Demonstrated positive approach to change
  • Ability to work independently in a fast-paced environment
  • Demonstrated knowledge of person-centred care approaches and principles
