Hours of Work:           Weekdays: Days; 8 hrs; As per schedule                          Summary of Duties:  The ALS Project Manager is an innovative nursing leadership opportunity with the primary goal to advance the recommendations and implementation plan arising from the ALS Models of Care Assessment, within Sunnybrook’s ALS Outpatient Clinic. The ALS Clinic is one of five regional ALS Clinics in Ontario, and includes an inteprofessional team and clinical research program.  The ALS Project Manager will report to Patient Care Manager of the Brain Sciences Program and will consult with the interprofessional team members, with a specific focus on 1) establishing an ambulatory nurse triage approach where the ALS nurse is the first point of contact for patient-initiated care inquiries between scheduled clinic visits, 2) establishing processes that enhance system partnership, 3)  building system capacity by advancing nursing and health professions knowledge about ALS care, 4) and developing and implementing an ALS pathway with role clarity that integrates collaborative responsibilities across sectors.   Key areas of focus for leadership and collaboration will include but are not limited to the following: •    Create an ambulatory nurse triage system for individuals with ALS from diagnoses to late stage disease, which integrates supportive care principles and facilitates navigation and connection to appropriate resources within, and external to, the ALS team. •    Enhance partnerships with community based teams, regional ALS Clinics and ALS Canada •    Provide leadership for the review of roles and responsibilities of the interprofessional team members  •    Evaluate and monitor success through key performance indicators •    Develop and implement an integrated person centered ALS care pathway that identifies role clarity with the ALS Clinic team providing consult to primary care, home care and community-based services that proactively anticipates the ALS disease trajectory and patient care needs •    Advance clinical excellence in ALS care collaboration, by designing and embedding educational processes that improve specialty knowledge for nurses and health professionals across sectors. The Program Manager will apply the principles of adult learning to achieve knowledge translation effectively and will ensure that all activities conducted are congruent with the organization’s policies, procedures, standards of safety and quality, and rooted in a person-centered philosophy. •    Support the development and distribution of education to patients and families for what to expect in ALS care across sectors •    Engage in knowledge translation of key initiative accomplishments and lessons learned Qualifications/Skill •    Current registration as a Registered Nurse (in good standing) with the College of Nurses of Ontario •    Master’s degree in Nursing an asset •    Experience in Project/ Change management, quality improvement and evaluation preferred •    Knowledge about neuromuscular disease process an asset •    Demonstrated ability to manage a diverse workload while setting priorities and meeting deadlines •    Ability to consult with stakeholders and integrate multiple perspectives to achieve project priorities •    Ability to perform duties in professional and courteous manner •    High level of initiative and self-direction •    Proven organizational, problem solving, time management, meeting facilitation and critical thinking abilities •    Ability to manage multiple, complex initiatives within tight timeframes •    Comfort in situations where the process to achieving goals is unclear •    Committed to on-going professional development
