Description
* Post-secondary education in a health related discipline relevant to the position or an equivalent combination of education and experience relevant to the position
* Working knowledge of provincial resources available for health care providers is an asset.
* Province of Manitoba Class 5 Drivers License, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health.
* Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology
* Demonstrated leadership and supervisory ability

POSITION SUMMARY:
The Physician Navigator is committed to the vision, mission, values and strategic priorities of Prairie Mountain Health, with a goal of reducing the administrative burden on physicians and providing more practice-specific support to physicians. The Physician Navigator is an ambassador for PMH and will interact with other medical providers and provincial allied health workers to improve physician navigation of available resources for their patients/practice, and be available to provide support to new physicians. The Physician Navigator functions in a confidential capacity in matters relating to labour relations.
RESPONSIBILITIES:
Overview:
1. Revise and continually audit/update the physician orientation processes, with an aim to improve physician knowledge of regional and provincial services.
2. Regularly tour the facilities/clinics to solicit feedback from the newer physicians for additional information that would have been helpful during orientation.
3. Be readily available for phone calls and in-person requests, but also maintain online support services.
4. Curate and maintain a directory of services in PMH in discussions with each clinic / hospital / manager / physician group.
5. Work with Diagnostic Services to develop an intimate knowledge of what tests are available, where they are available and how to order them.
6. Ensure all computer access is ready upon start date.
7. Provide a broad overview of PMH services, through the lens of the specific community.
8. Arrange dedicated orientation time for new physician with Department Chief/Site CMO/Clinic physician.
9. Work with communities who are receiving new physicians to curate a list of local resources, including family support information.
10. Assist with regular check-ins with new physicians to the region.
11. Regularly tour the facilities/clinics to solicit feedback from established physicians to discuss the patient navigation issues.
12. Regularly perform exit interviews and report to Executive Management with noticeable issues and/or patterns.
13. Define what specific procedures are available in PMH.
14. Define the referral process for PMH as well as Winnipeg.
15. Define what clinical issues could be dealt with virtually.
16. Define which services need a physician referral vs those where patients can self-refer.
17. Define the community resources available and how to access them
18. Create communications to physicians about new/updated services as they become available.
19. Other duties as assigned
