Nurse Practitioner - Nurse Lead Outreach Program

The Nurse Practitioner within the Nurse-Led Outreach Team (NLOT) program utilizes advanced practice nursing expertise to provide comprehensive clinical care. This includes advanced assessment, diagnosis, and treatment, as well as education, consultation, and research in the field of Geriatrics. The position plays a vital role in improving access to timely, evidence-based medical care for residents in long-term care homes (LTCHs) within the Mackenzie Health Hospital catchment, decreasing avoidable ED transfers and hospital admissions, and advancing a more integrated, resident-centered model of care delivery. This role will positively impact organizations, improving system partnerships, increasing capacity and efficiency through decreasing wait times and length-of-stay, and facilitating enhanced information exchange.

Contribute to safe, quality care by

  • Understanding scope of practice of the nurse practitioner along with responsibilities and accountabilities.
  • Consulting with and/or referring clients to other health-care providers at any point in the care continuum when the client’s condition is not within the nurse practitioner’s scope of practice or the individual nurse practitioner’s competence.
  • Advocating for clients in relation to therapeutic interventions, health care access, the health care system and policy decisions that affect health and quality of life.
  • Collaborating with members of the health care team to provide and promote interprofessional client-centered care at the individual, organizational and system levels.
  • Contributing to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.

What must you have?

  • Master’s Degree in Nursing
  • Current certificate of competence from the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care (PHCNP) or Nurse Practitioner-Adult (NP-Adult)
  • Minimum of 2 years of current expertise in the care of the adult medical population

  • Valid driver’s license and availability of a car.
  • Demonstrated good attendance record with the ability to maintain this same standard.
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required.
  • Proven attendance record.
  • Ability to perform the essential duties of the job.

Preferably, your profile also includes:

  • National certification in Emergency and / or Gerontology Nursing from the Canadian Nurses Association.
  • Membership in Registered Nurses’ Association of Ontario and Nurse Practitioner Association of Ontario.
