Position Summary
We are hiring a permanent, part-time (0.80 FTE) registered Social Worker in the Castlegar’s Primary Care Network.
Days of work: Monday to Thursday
Shift times: 08:30 to 16:30

About this position:
This is a primary care network social work position supporting the Castlegar Medical Clinic. As part of the Primary Care Network model, you will be integrated into a collaborative environment that is committed to community wellbeing and guided by respect, patience, and strong support for both patients and staff. The clinic provides family medicine and community based health services to individuals and families within Castlegar and the surrounding region.
In this role, you provide comprehensive therapeutic clinical counselling services to patients and families within the assigned clinical specialty area. You conduct thorough social work and psycho social assessments using a variety of modalities, following professional standards as well as clinical policies and practices. You educate and offer consultation to patients, families, and healthcare professionals. You will offer leadership to the interdisciplinary team by contributing to care planning, ensuring the effective use of patient resources. This position participates on local committees and planning bodies to identify opportunities to improve efficiency and effectiveness in service delivery, enhance patient care, and reduce gaps within the community. You develop, coordinate, and deliver in services, presentations, psycho educational sessions, and support groups. You also initiate, design, and carry out research activities in collaboration with the interdisciplinary team.
Hours and days of work and work location are subject to change. Access to a personal vehicle may be required.

Qualfications
Masters Degree of Social Work from an accredited school of Social Work. One year’s recent related experience or an equivalent combination of education, training and experience. Current full registration with the British Columbia College of Social Workers. Current valid BC Driver’s License required.
