Position Summary
Cranbrook Urgent and Primary Care Centre (UPCC) is hiring a relief, full time Primary Care Social Worker.
The Cranbrook Urgent and Primary Care Centre (UPCC) provides team-based care for non-emergency health concerns to individuals in Cranbrook that require same day attention. Care is available for concerns such as minor injuries, sprains and strains, infections, less serious child illness and injury, high fever, and cuts, wounds or skin conditions.
About the role:
Primary Care Social Workers have a key role in integrated healthcare by addressing emotional, social, and practical challenges that impact patient health. You’ll collaborate with physicians, nurses, and allied health professionals to support care plans, improve access to medical and community resources, and enhance patient outcomes. From mental health and crisis intervention to housing, food security, and benefit navigation, you’ll help patients manage complex needs while promoting holistic well-being within an urgent and primary care setting. Empower clients by providing counseling, group work, advocacy, and system navigation while connecting them to specialized resources.
Days of work: Monday to Friday; 08:30 to 16:30. This is a temporary opportunity until April 2026, or return of incumbent. Once hired into a relief role, employees are eligible to apply for internal postings including permanent positions. There are many opportunities with Interior Health that begin with relief positions and lead to regular employment.

Qualfications
Bachelor’s Degree in Social Work from an accredited university. Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience. Current full registration with the British Columbia College of Social Workers. Current valid driver’s license.
