Now Hiring Home Health Transformation Physiotherapist!
Are you passionate about delivering client-centered physiotherapy that makes a real difference in people’s lives? Join our innovative Home Health Transformation team in Creston, where you’ll provide coordinated, accessible care to help clients live independently and confidently in their own homes.

As a Physiotherapist on this team, you’ll be at the forefront of this transformation, delivering personalized care that enhances mobility, function, and quality of life-right where clients feel most comfortable.
Your Impact
• Conduct thorough assessments to understand each client’s unique needs and goals.
• Develop and implement tailored physiotherapy programs that boost functional independence and promote health at home.
• Provide expert advice on equipment needs and prescribe assistive devices to support client safety and mobility.
• Educate clients and caregivers on self-management strategies and injury prevention.
• Collaborate closely with interdisciplinary healthcare teams, leveraging digital tools to enhance communication and continuity of care.
Work-Life Balance & Schedule
Enjoy a varied workweek with a consistent schedule of rotating days - 08:00 AM to 4:00 PM
In addition to the scheduled hours, this position includes on-call hours from 04:00 PM to 10:00 PM, in the following rotation:
• Week 1: Tuesday & Wednesday
• Week 2: Monday, Friday through Sunday
• Week 3: Thursday
A valid driver’s license is required as you will travel to clients’ homes using your personal vehicle (with mileage reimbursement) or an assigned fleet vehicle.
