ROLE
The Patient Flow Navigator is a leadership position that plays a pivotal role in supporting and optimizing patient flow across the Huron Perth Healthcare Alliance (HPHA). Working in close collaboration with Team Leads, Bed Allocators, Allied Health, Physicians, and the Patient Flow Manager, this role ensures smooth and timely transitions of care while maintaining a patient- and family-centered approach.
The successful candidate will coordinate daily patient flow operations and ensure efficient, high-quality care delivery across all units day and night. In alignment with local and provincial policies and strategies, the Patient Flow Navigator will implement and monitor initiatives to reduce inpatient length of stay, improve access, and enhance inpatient bed utilization across HPHA’s facilities. The Patient Flow Navigator will act as the first contact for critical decision making off hours and on weekends in collaboration with AOC. A collaborative leadership approach will occur during regular working hours with team leaders, managers and Patient Flow Navigator.
Additionally, this role carries accountability for staffing oversight during their shift including:
- Approving short call staffing needs
- Coordinating staffing adjustments
- Reassigning staff as necessary to meet unit acuity and dynamic patient needs
- Planning and maintaining effective 24-hour patient flow coverage
Key Responsibilities:
- Provide leadership for patient flow across HPHA during assigned shifts and on call based on internal pressures ; act as the escalation point for flow and capacity decisions when required
- Collaborate with interprofessional teams to facilitate timely admissions, discharges, and transfers
- Proactively identify and resolve patient flow barriers
- Reassign staff to meet fluctuating acuity and census across units
- Approve and oversee short call staffing decisions in collaboration with unit leadership and scheduling
- Implement staffing adjustments in real-time to support safe, quality care delivery
- Develop, communicate, and maintain strategic patient flow plans over a 24-hour period
- Ensure alignment with HPHA policies and broader provincial health strategies including the ALC Leading Practices Guide, P4R Initiative, and Ontario Health data findings
- Support discharge planning efforts in collaboration with community partners and internal care teams
- Promote a culture of safety, collaboration, and excellence in care transitions
REQUIRED EDUCATION & CERTIFICATION(S)
- Registered health professional with current registration in the appropriate Ontario regulatory body (RN/RPN, RRT, Social Work, etc) required
- Post secondary diploma/degree in related health profession required
- Current Basic Life Support (BLS) certificate required
- Crucial Conversations or willing to obtain
- Emotional Intelligence course or willing to obtain
REQUIRED SKILLS & EXPERIENCE(S)
- Minimum of two years’ experience in care coordination, discharge planning, or patient flow in an acute care or complex care environment
- Knowledge of community, government resources and legislation relevant to discharge planning and alternate level of care (ALC)
- Demonstrated leadership, clinical assessment and decision-making skills appropriate to a high acuity, fast paced environment
- Excellent communication (written and verbal), negotiation and conflict resolution skills
- Strong organizational skills with the ability to prioritize multiple competing demands while maintaining patient centered care
- High emotional intelligence, cultural sensitivity and ability to build therapeutic relationships with patients, families and interdisciplinary teams
- Ability to interpret and act on operational and patient flow data (ALC reports, bed availability, throughput metrics)
- Familiarity with electronic health systems (Meditech or equivalent), bed management dashboards (Pulse/Start Hub), and scheduling platforms (UKG or similar) - ability to liaise and work with the Scheduler on staffing changes
- Ability to work independently and collaboratively within an interprofessional team
- Commitment to equity, diversity and inclusion principles in patient care and team interactions
DESIRED SKILLS, EXPERIENCE(S) & EDUCATION
- Recent (within last 5 years) experience with patient flow is preferred
- 2 years’ recent leadership or charge experience is an asset
- Familiarity with Quality Improvement methodologies (e.g., PDSA) is an asset
