At Humber River Health the Allied Health team plays a pivotal role delivering diagnostic, technical, supportive and therapeutic patient care. Occupational Therapists are valued members of our Allied Services Department. 

 

Our Occupational Therapists provide evaluation, treatment, and education to assist patients in doing things with the abilities they have, and help to facilitate their activities of daily living, including the physical and cognitive aspects of their recovery. Reporting Relationship: Manager, Allied Health Services

Hourly Rate:  $38.84 - $48.22 per hour

Employee Group: OPSEU 590

Site/Location: Primary assignment is at the Wilson site.

Availability:  Available to work rotating shifts (Monday to Friday, Weekends & Statutory Holidays)

 

 

Position Responsibilities:

  • Providing appropriate assessment and treatment of referred frail senior patients through the new Assess and Restore Program (HEART)
  • Providing comprehensive screening and consultation of referred frail senior population through the Emergency Department
  • Provides health education for patients and families regarding chronic disease management, diagnostic tests and prescribed treatments
  • Integrates best practice guidelines and evidence-based research into clinical practice related to Seniors Care
  • Maintains abreast of new developments in geriatric syndrome managements and Seniors Care with integration of related evidence-based advancements into clinical practice
  • Liaises with the patient’s primary care practitioner and community agencies as required throughout the care process
  • Prioritizing patient caseload to meet the needs of referred patients
  • Recording timely workload measurement data
  • Recording client assessment, progress and discharge status
  • Provide consultation to other disciplines in the area of physical medicine for Seniors Care
  • Utilizes Resource Matching and Referrals (RM&R) and other referral sources to ensure patients are linked to appropriate resources
  • Works as a member of the health care team in collaboration with the ED and Inpatient Programs
  • Develops linkages with external partners as appropriate to ensure continuity of care
  • Flexibility in caseload and program assignment
  • Able to manage patient caseload in a busy acute care setting
  • Clinical supervision of Rehabilitation Assistants according to the College of Occupational Therapists of Ontario’s support personnel standards
  • Clinical supervisor of students, including Occupational Therapist and/or OTA/PTA students
  • Travel and cross coverage to both the Wilson and Finch locations may be required.
  • Access to a vehicle for cross site coverage will be an asset
  • Position will require scheduled weekend work, evenings, and statutory holiday coverage

 

Qualifications:

  • Bachelors or Masters Degree in Occupational Therapy from an accredited program
  • Current registration in good standing with the College of Occupational Therapists of Ontario required
  • Membership in CAOT preferred
  • 5-7 years previous Occupational Therapy experience is preferred. Experience working with the acute care setting and the frail elderly population required
  • Community experience and knowledge of community resources is required
  • Demonstrated ability to manage a variety of tasks and work efficiently
  • Demonstrated excellent written, oral, and verbal communication skills
  • Demonstrated ability to act as a valuable part of an interdisciplinary team
  • Demonstrated excellent interpersonal skills
  • Demonstrated flexibility, adaptability, creativity and resourcefulness
  • Able to work independently
  • A positive attendance and discipline record
  • Availability to work during both the week, weekend, and evenings required
  • Access to a vehicle and ability to travel from site to site is an asset
