Job Description: Under the direction of the Manager (or designate), and as part of the interdisciplinary Team, the Social Worker- MSW is responsible for the full scope of medial social work services for older adults experiencing frailty and complex comorbidities. This is accomplished through collaboration with the patient, family, and other members of the interdisciplinary team. The Social Worker - MSW is responsible to develop a professional therapeutic working relationship with patients and families, including an understanding of the social work role, and SHA philosophy, objectives, and policies. The Social Worker - MSW utilizes a range of interventional skills from a number of theoretical models and focuses on direct patient care. The core functions of geriatric social work practice include biopsychosocial assessment, care coordination, caregiver support, crisis management, advocacy, advance care planning, resource navigation, benefits application and discharge planning.
