Job description: The Social Worker - BSW Senior, reporting to the Manager, or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals and consultations to appropriate services and provides on-site and virtual social work services.

Experience
- 2 years SW experience working with issues of capacity, dementia, grief, and loss
- Ability to establish and maintain effective interpersonal skills
- Requires highly developed clinical assessment skills and critical thinking
Education
- Bachelor of Social Work (BSW)
Other Education and Training
- Experience in medical social work is an asset
