Job description: 

The Social Worker - BSW Senior, reporting to the Manager, or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals and consultations to appropriate services and provides on-site and virtual social work services.

Education:

  • Bachelor of Social Work (BSW)

Experience:

  • Ability to establish and maintain effective interpersonal skills
  • Requires highly developed clinical assessment skills and critical thinking
  • 2 years SW experience working with issues of capacity, dementia, grief, and loss

  • Ability to conduct client assessments and engage in effective treatment planning
  • Ability to demonstrate initiative, independent judgment & organizational skills
  • Ability to work under stress
  • Sound judgment, tact, initiative and high standards of integrity/responsibility
