Job Description: The Senior Social Worker is an experienced clinician who may provide supervision and coaching within acute care in the Discharge Planning and Support. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to assess and support patients and/or to coach, mentor, orientate and provide education to an assigned team in Discharge Planning and Support. Drawing on standards of practice for case management; professional codes of ethics; social work standards of practice in health care; program policy and procedures; and, relevant legislation, the senior social worker provides high quality client service and maintains professional standards of practice. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"
