Job Description

Reporting to the Care Team Manager the Resident Care Coordinator is responsible for supporting the highest standard of care and service provision to residents living on a given neighborhood. As an experienced practitioner, the resident care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and provide support and teaching to the resident care team (including residents and residents' families) within a collaborative environment. The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.

Job Qualifications

Required Qualifications

  • Bachelor Degree in Nursing
  • Current BCLS certification
  • Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)

Experience
Current knowledge and a minimum of three (3) years recent experience in long term care nursing within a team environment is required.

  • Demonstrates ability to facilitate the implementation, evaluation, and ongoing design of the current client centered care model.

  • Demonstrates critical thinking, ability to problem solve, utilization of conflict resolution skills and decision-making abilities.
  • Demonstrates effective verbal and written communication skills.
  • Demonstrates excellent knowledge of the long-term care needs of adults and their families, and a practical application of a broad range of clinical interventions.
  • Demonstrates leadership abilities.
  • Demonstrates self-direction.
